We are in-network with most major insurance providers.
We accept claims related to Motor Vehicle Accidents (MVA) and Workers’ Compensation.
We accept a limited number of Oregon Health Plan (OHP) Medicaid plans — please refer to the list below for details.
We do not provide superbills or documentation for out-of-network reimbursement.
If you are uncertain about your insurance coverage or benefits, we are happy to verify your eligibility upon request.

We offer a discounted cash-pay rate for patients who pay at the time of service.
An additional 25% discount off our time-of-service rates is available for:
Kaiser ChooseHealthy program members
Patients experiencing financial hardship who complete a Financial Assistance Application

Good Faith Estimate Notice
Your Right Under the No Surprises Act
If you are uninsured or choosing not to use insurance, you have the right to receive a Good Faith Estimate explaining the expected cost of your medical or mental health care.
You may request a Good Faith Estimate before scheduling any service.
If you receive a bill that is $400 or more above your estimate, you have the right to dispute the charges.
